1. Managing Drug Related Problems DRPs

A Drug-Related Problem is an event or circumstance involving drug therapy that actually or potentially interferes with desired health outcomes. For the use of the PCNE classification it is important to separate the real problem (that affects or is going to affect the outcome) from its cause

2. Patient Counseling Service

Patient counseling is a key competency element of the Pharmaceutical Care process. Given the advertising for medication in the media and easy access to information on the Web, it is important for pharmacists to provide appropriate, understandable and relevant information to patients about their medication.


3. Treatment Chart Review

Chart-review studies define medication errors as errors in the ordering, transcribing, dispensing, administering, or monitoring of a medication. Potential ADEs are defined as errors that have the potential to harm a patient.On the other hand, by definition, preventable ADEs are associated with errors.

4. Medication Reconciliation

Medication reconciliation is the process of creating the most accurate list possible of all medications a patient is taking — including drug name, dosage, frequency, and route — and comparing that list against the physician's admission, transfer, and/or discharge orders, with the goal of providing correct medications.

5. Formulary Management

Prescription medication coverage is one of the most expensive components of a health plan for a state agency or health care organization. When a new medication or product becomes available, our clinical staff conducts a thorough evaluation process to determine if we should place the new product on the formulary list or if it should remain a non-preferred drug.

6. Drug Utilization Review DUR

Drug utilization review (DUR) is defined as an authorized, structured, ongoing review of prescribing, dispensing and use of medication. DUR encompasses a drug review against predetermined criteria that results in changes to drug therapy when these criteria are not met.

7. Therapeutic Drug Monitoring TDM

Therapeutic drug monitoring (TDM) is a branch of clinical chemistry and clinical pharmacology that specializes in the measurement of medication concentrations in blood. Its main focus is on drugs with a narrow therapeutic range, i.e. drugs that can easily be under- or overdosed.

8. ADR Reporting

The Pharmacovigilance Program of India (PvPI) was launched with a broad objective to safe guard the health of 1.27 billion people of India. Adverse drug Reactions (ADRs) are reported from all over the country to NCC-PvPI, which also work in collaboration with the global ADR monitoring centre (WHO-UMC), Sweden to contribute in the global ADRs data base. NCC-PvPI monitors the ADRs among Indian population and helps the regulatory authority of India (CDSCO) in taking decision for safe use of medicines.
