Over the next 4 weeks, you will participate in a self-learning exercise designed to help you improve your clinical practice. First, you will assess how effective you are in evaluating your patients’ barriers and adherence to medications in your current clinical practice. On the attached pages are prepared spreadsheets for you to document whether various medication use evaluations and clinical practice activities were performed on each patient you see. During your continuity clinic, fill out one column in the spreadsheet for each patient you see. Many of these items may not pertain to the present clinic visit but can be completed by performing a quick chart review for your patients. Some answers may require consultation with the patient’s pharmacy. You must document at least 3 patients. You will then review your results and evaluate how you can improve your practice and design an action plan to do so for the year.


Finally, you will choose one patient whose chart you have reviewed to be scheduled for a consultative session with you and the clinical pharmacist. During this visit, you should address the areas for improvement that you have identified and use feedback from the patient and the pharmacist to continue to help your patient manage their medications.


To best provide you with feedback during the visit with your patient and the clinical pharmacist, the patient, clinical pharmacist and you will evaluate you in various areas. These will include your interpersonal and communication skills, professionalism, ability to inform and educate your patient, how well you provide emotional support and help alleviate their fear and anxiety, and your respect for your patient’s values and preferences. We will share all evaluation forms with you ahead of the visit.


We hope that you find this exercise to be an innovative way to help your patients manage their various medications with the close assistance of a clinical pharmacist, while providing you with useful feedback on your skills.
