A shared care guideline outlines how responsibilities for managing a medicine 's prescription can be shared between the specialist and a primary care prescriber.

Shared care agreements outline the specialist/GP/patient responsibilities when shared care drugs are initiated, and the patient stabilized on the drug, by the specialist and then the patient's GP is asked to continue prescribing.
