2.drug interactions :digoxin and warfarin _loss of anticoagulant control .
this is a major pharmaceutical problem

3.patient is dehydrated % also suffering from uti,so she should drink lots of fluids,
digoxin should be taken for only 5 days a week.
avoid chances of bleeding ,
using equipments for urinary tract should be a sterilised one .

4. vit k in inpatient list
5. anticoagulant drug warfarin cause bleeding and increased INR

14. some medicines will interact with the feed resulting in reduced absorption,so based on this the feed should be altered
oral medicines should not be drawn into IV syringes,as it leads to wrong route errors

6.digoxin dose for atrial fibrillation:
dose: iv 8 to 12 mg mcg/kg
tablets 10 to 15 mcg/kg
oral solution 10 to 15 mcg/kg

maintanence dose: iv 2.4 to 3.6 mcg /kg
tablets 3.4 to 5.1 mcg/kg
orl solution 3.0 to 4.5 mcg/kg

here the maintenece dose given is too high(625 mcg)...should reduce it.(avg weight 60:< 300-350 mcg > reccomended.

7.warfarin and vit k has interactions..effct of warfarn will be affected.
also warfarin and digoxin has serious interactions. so warfarin can be changed from the prescription
