Looking for a for one of the leading Hospitals of HealthCare Global Enterprises Ltd & position based at Kolkata (West Bengal). Details are as follows:

Education: Doctor in Pharmacy (Pharm D)

Experience Range: Minimum 2 years in oncology

Compensation: Best in the industry

Roles & Responsibilities

Documentation & reporting of Clinical Research of the respective unit/establishment.

Documentation & monitoring on Chemotherapy administration & proportion of medical charts to be reviewed by clinical pharmacologist within 24 hours of admission.

Reporting of Adverse Drug Reaction
Monitor & track any uncommon side effects seen in any patient, after giving chemotherapy drugs, which should be recorded by the clinical pharmacologist.

Patient counseling: Proportion of patients who have had a face-to-face discussion about medicines-specific information & proportion of patients who receive education from a Clinical pharmacologist about their disease(s) and medications(s) during their hospital stay & during discharge.

Adherence to national/International guidelines for chemotherapy protocols, administration & monitoring.
To design, conduct, analyze and report on clinical pharmacology studies.

To Conduct, Analyze & Check Chemotherapy order forms & drugs before dispensing with prescription.

To Monitor & Ensure Patients undergo regular chemotherapy processes.

Supervising all chemotherapy drug mixing processes & mixing whenever required.

Prescribing Errors - Identification and resolution of unintentional departure from recommended prescribing practices per patient bed day & Identification and resolution of potential or actual drug-related problems per patient bed day.

Prescription Audit to be Conducted
Establish quality of Service by establishing and following good Organizational and Ethical Standards.
The proportion of patients who receive education from a Clinical pharmacologist about their disease(s) and medications(s) during their hospital stay

Reporting of Quality Indicators as per NABH Guidelines
Liaison for Ethics Committee
